EMERGENCY ARCHITECTURAL SERVICES WANTED Applications for

PAPER ATTACHED. THIS STANDARD FORM LASB 1 - 2003 EDITION HAS AN INSTRUCTION

OTHERWISE NOT FOLLOWING THIS FORMAT WILL BE DISCARDED. 1. Hurricane Katrina
Repair Work to Willowbrook Apartments - LA Housing Authority - 7001 Bundy Road,
New Orleans, LA, Project No. 01-107-05B-13, Part 05. This project consists of
the restoration of a 408 unit multi-family housing rental complex. The complex
includes 25 three story apartment buildings and 5 support buildings totaling
approximately 300,000 square feet. These buildings sustained substantial storm
damage from wind driven rain and flooding particularly to the ground floors.
Grounds floors will require replacement of wall finishes, flooring ceilings,
bathroom and kitchen cabinets and fixtures. The upper floors will require
carpet replacement, cleaning and painting. The mechanical and electrical
systems will need to be evaluated and repaired or replaced where appropriate.
The Designer will propose potential mitigation measures to prevent future
damage. Environmental remediation is not part of this contract. Funding for
this project is provided by the Federal Emergency Management Agency and their
participation and oversight maybe expected in all phases of the project. The
designer shall prepare and submit all required drawings to Facility Planning
and Control in AutoCAD Release 2000 or lower on 3.5" DS, HD diskettes or
CD ROM. Files may be compressed (PKZIP) but if so, provide software to
uncompress (PKUNZIP). Drawings shall follow the format specified in the
"Instructions to Designers for AutoCAD Drawings Submittal". The funds
available for construction are approximately $11,000,000.00 with a fee of
approximately $875,065.00. Contract design time is 120 consecutive calendar
days; including 40 days review time. Thereafter, liquidated damages in the
amount of $900.00 per day will be assessed. Further information is available

REQUIREMENTS APPLICABLE TO ALL PROJECTS: Applicants are advised that design
time ends when the Documents are "complete, coordinated and ready for
bid" as stated in to Article 3.3.1 4) of the Capital Improvements Projects
Procedure Manual for Design and Construction. Documents will be considered to
be "complete, coordinated and ready for bid" only if the
advertisement for bid can be issued with no further corrections to the
Documents. Design time will not necessarily end at the receipt of the initial
Construction Documents Phase submittal by Facility Planning and Control. Any
re-submittals required to complete the documents will be included in the design
time. In addition to the statutory requirements, professional liability
insurance covering the work involved will be required in an amount specified in
the following schedule. This will be required at the time the designer's
contract is signed. Proof of coverage will be required at that time. SCHEDULE

When these projects are financed either partially or entirely with Bonds, the
award of the contract is contingent upon the sale of bonds or the issuance of a
line of credit by the State Bond Commission. The State shall incur no
obligation to the designer until the Contract Between Owner and Designer is
fully executed. Firms will be expected to have all the expertise necessary to
provide all architectural services required by the Louisiana Capital
Improvement Projects Procedure Manual for Design and Construction for the
projects for which they are applying. Unless indicated otherwise in the project
description, there will be no additional fee for consultants. ANY PERSON

SPECIAL ALERT: HURRICANE KATRINA RELIEF EFFORT
A DESIGN-BUILDER FOR DESIGN BUILD SERVICES AND TO SOLICIT LETTERS
OF INTEREST AND STATEMENTS OF QUALIFICATIONS FOR A NEW LOUISIANA NATIONAL GUARD
BARRACKS AT JACKSON BARRACKS, NEW ORLEANS, LA As authorized by the State of
Louisiana Revised Statute 29:42 (R. S. 29:42) and approved by the Commissioner
of Administration of the State of Louisiana, the Adjutant General and the
Military Department of the State of Louisiana hereby give notice of intent to
select a design-builder for design build services for the Barracks, Jackson
Barracks, New Orleans, Louisiana. Project Description: This project is to
construct a new barracks and renovate existing barracks totally approximately
41,600 SF at Jackson Barracks, LA to replace the facilities destroyed during
Hurricane Katrina. Project value is approximately $19,600,000. Specially
designed barracks facilities with exterior walls of brick with concrete masonry
unit backup or other suitable systems; walls and partitions of drywall, block,
or other economically suitable material; concrete floors; metal standing seam
roof or hip or gable type construction (metal standing seam); and flag pole. A
multi-story structure or structures with ventilation, heating, and air
conditioning. Supporting facilities include privately-owned vehicle parking,
fencing, sidewalks, exterior fire protection, outside lighting, access roads,
detached facility signs, utilities, and storm water retention pond(s). Physical
security measures will be incorporated into design including maximum feasible
standoff distance from roads, parking areas, and vehicle unloading areas, heavy
landscaping, and bollards to prevent access when standoff distance cannot be
maintained. The new facility shall also require Commissioning for SPiRiT based
"Gold" level rating. Cost effective energy conserving features will
be incorporated into design, including energy management control systems and
high efficiency motors, lighting, and HVAC systems. Existing Barracks will be
demolished as part of this project. R. S. 29:42 requires every design-builder
to be licensed and registered in the state as either an architect, an engineer,
or a general contractor. Each design-builder may do any of the following: (1)
Sublet responsibility for professional design services to an individual, firm,
or corporation licensed and registered in the state to provide professional
design services. (2) Sublet responsibility for construction or other services
requiring a contractor's or trade subcontractor's license to persons or
entities registered, licensed, or otherwise qualified to provide those
services. (3) Contract with the Military Department as part of a design-build
contract to provide professional services or construction services that the
design-builder is not licensed, registered, or otherwise qualified to
undertake. If the design-builder is not licensed, registered, or otherwise
qualified as noted but does meet the requirements of 29:42 as being licensed
and registered in the state as either an architect, an engineer, or a general
contractor , then the design-builder must sublet those responsibilities for
which he is not qualified to a licensed/registered entity as outlined in
paragraphs (1) and (2) above. Design-builders who want to be considered for
selection for this project are requested to complete and submit the response
form and a statement of qualifications by April 14, 2006 to: LTC William Aldridge,

or the Dodge Plan Room. Any response failing to meet the requirements outlined
in this notice of intent will not be considered by the department. False or
misrepresented information furnished in response to a notice of intent shall be
grounds for rejection by the department. As required by R. S. 219:42, the
Construction and Facilities Management Officer (CFMO) will appoint a primary
design-build evaluation committee. The primary design-build evaluation
committee will evaluate the responses to the notice of intent and will select a
short list of no fewer than three and no more than five of the highest rated
entities. However, if fewer than three responses are received, the division of
administration shall have discretion to authorize the departments to proceed
with the design-build process. Phase 1 Evalution Criteria The following general
criteria will be used by the primary evaluation committee in evaluating
responses to this notice of intent for design-build services shall apply to
both the design and construction components of any responding entity.
Information provided by respondents to this notice should be separated into
design and construction sections: (a) Experience of both the design and
construction entity components and of key personnel as related to the project
described in this notice. Offerors should provide recent evidence of relevant
past performance for projects within the last three - (5) years. NOTE: PROJECTS
SUBMITTED IN PAST PERFORMANCE DO NOT NECESSARILY HAVE TO BE DESIGN BUILD
PROJECTS. (a) 1.1 Experience of Design Firm Submit past performance narratives
for the design firm(s) for three (3) to five (5) projects that demonstrate
design experience in performing work similar to that described in the "Project
Description". Include design awards, customer letters of commendation,
etc., with points of contact and telephone numbers. A list of relevant project
information might include the following information. " Project name and
location " Project Description " Nature of firm's responsibility
(design, construction or both) " Identify type of contract (design,
design/build, or construction) " Project owner's name and address and
project manager's point of contact, name, telephone number, fax, and email address
(if known) " Date started " Original scheduled completion date "
Actual completion date " Overall size of project (in square feet or square
meters) " Construction cost (excluding design costs) " Duration of
construction (excluding design time) " Problems encountered and corrective
actions taken " Relevance of experience to the solicitation project "
Copies of recent performance evaluations (if available) for similar projects
Additionally, describe up to three (3) partnering/teaming arrangements with construction
teams. Identify proposed key personnel who directly worked on referenced
partnering/teaming arrangements. (a) 1.2 Experience of key design personnel
Submit key design personnel staffing that demonstrates technical qualifications
in all disciplines required to perform work similar to that described in the
"Project Description". All lead designers must be registered /
certified in the State of Louisiana in their discipline. (a) 2.1 Experience of
Construction Firm Submit past performance narratives for the construction firm
for three (3) to five (5) projects that demonstrate construction experience in
performing work similar to that described in the "Project
Description". Include awards, customer letters of commendation, etc., with
points of contact and telephone numbers. A list of some relevant project
information might include the following information. " Project name and

management personnel of the Construction Firm v. History of the design and
construction components working together. (b) Past performance of State of
Louisiana Military Department a. Quality performance b. Schedule performance c.
Cost performance The State of Louisiana National Guard will evaluate the
quality of the offeror's past performance. The assessment of the offeror's past
performance will be used as a means of evaluating the relative capability of
the offeror to successfully meet the requirements of the contract. The State of
Louisiana National Guard reserves the right to obtain information for use in
the evaluation of past performance from any and all sources including sources
outside of the State of Louisiana National Guard. Offerors lacking relevant
past performance history will not be evaluated favorable or unfavorable for
past performance. However, the proposal of an offeror with no relevant past
performance history, while rated neutral in past performance, may not represent
the most advantageous proposal to the State of Louisiana National Guard and
thus, may be an unsuccessful proposal when compared to the proposals of other
offerors. (c) Past performance on similar projects performed for other clients
Past performance will be evaluated as outlined in (b) above. (c) 1. Other
National Guard (other States) or DOD clients (c) 2. Corporate or Commercial
clients (d) Capacity to perform the requirements of this specific project (d)
1. Capacity to perform the work within schedule (d) 1.1 Capacity of the
Construction Firm (Management and self-perform functions) (d) 1.2 Capacity of
Trade Subcontractors (d) 1.3 Capacity of Design Firm (d) 2. Bonding capacity
Phase 1 Evaluation Guide EXCEPTIONAL (E) The proposal exceeds the requirements
of the NOI and provides an exceptional or outstanding approach, which fully
satisfies the Government's requirements. A complete understanding of the
solicitation is demonstrated. Selection for Phase II may be made without
exchanges with the offeror. ACCEPTABLE (A) The proposal fully satisfies the
requirements of the NOI and demonstrates a good understanding of the solicitation.
The offeror has adequately addressed all of the technical elements requested by
the NOI. MARGINAL (M) The proposal does not fully meet the requirements of the
NOI. Weaknesses are identified which would indicate an insufficient
understanding of the NOI requirements. With minor revisions or clarifications,
the proposal has a reasonable chance of becoming technically acceptable. POOR
(P) The proposal fails to satisfy requirements of the NOI and the approach
contains an unacceptable level of risk to the Government. Major deficiencies
have been identified in the proposal, which are either not correctable or would
require major revision/rewrite to the proposal, without which the proposal
doesn't have a reasonable chance of becoming technically acceptable. The short-listed
entities will be invited by the Adjutant General to submit a detailed technical
and cost proposal for the design-build project. As required by R. S. 29:42, a
Technical Review Committee will evaluate the design-build proposals. The
Technical Review Committee shall identify specific technical elements of the
project to be included in the technical score. The scores of assigned elements
shall be considered public records and shall be available for public review.
The Technical Review Committee will include representatives from the
construction, building design, and planning sections of the department. The
project manager assigned to the project, will be the chairman of the Technical
Review Committee for the project. The Technical Review Committee, including the
project manager, shall identify specific technical elements of the project,
depending upon the project characteristics. Additionally, the CFMO, with the
concurrence of the Adjutant General, may select additional department
engineering and technical experts, and nationally recognized design-build
experts to serve as committee members to score each technical element of the
project. Members of the Technical Review Committee shall not have served as
members of the primary evaluation committee. Each member of the Technical
Review Committee shall make his scoring of assigned elements available for
public review. Such scores shall be considered public record. R. S. 29:42
requires that an adjusted score approach to be used in determining the winning
proposal. Prior to determining the adjusted score, each design-build proposer
shall be notified, in writing, of its final technical score. A proposer may
request, in writing, no later than 10 business days from the date of the
chairman's notice, a review of its final technical score. An adjusted score
shall be determined using the following three components: (a) (i) The technical
score will be determined by the Technical Review Committee. Weighing factors
may be assigned to each element depending on its relative magnitude or
significance to the overall project. Each Technical Review Committee member
shall rate his assigned element of the proposal from each of the entities on
the short list and shall submit such scores to the chairman of the Technical
Review Committee. The schedule and price bid shall not be made known to the
Technical Review Committee during the scoring process. The chairman of the
Technical Review Committee shall adjust the scores for any applicable weighing
factors and shall determine the total technical score for each proposal. (ii)
Prior to determining the adjusted score, the chairman of the Technical Review
Committee shall notify each design-build proposer, in writing, of each
proposer's final technical score. A proposer may request, in writing, no later
than ten business days from the date of the chairman's notice, a review of its
final technical score by the CFMO or his designated representative. If any
proposer requests a review of its total technical score, the CFMO shall hold a
hearing to review such within a reasonable time after the request has been
received by the CFMO. The CFMO shall give the requesting proposer reasonable
notice of the time and place of such hearing. The requesting proposer may
appear at the hearing and present facts and arguments in support of the request
for review of its final total technical score. (iii) The CFMO shall present his
findings from the hearing to the Adjutant General. The Adjutant General shall
determine what action shall be taken regarding the proposer's request to review
its final technical score. Except as provided for in R.S. 48:250.2(D), the
Adjutant General's decision shall be final and not subject to appeal by any
legal process. (b) The time value, consisting of the product of the proposed
contract time expressed in calendar days multiplied by the
value-per-calendar-day expressed in dollars established by the department and
included in the "scope of services package". (c) The price proposal.
R. S. 29:42 provides that the winning proposal shall be the proposal with the
lowest adjusted score. The winning proposal shall be the proposal with the
lowest adjusted score. The adjusted score for each entity's design-build
proposal shall be determined by the following formula: adjusted score = (price
bid + time value) divided by the technical score. If the time value is not
used, the adjusted score shall be determined by the following formula: adjusted
score = price bid divided by technical score. Questions regarding this notice
