NOTE: Applications will be valid for six months beginning on the submittal date stated above or as extended. If there is a continued need for these services after the applications have expired, Facility Planning and Control will issue another advertisement. Facility Planning and Control will require professional architectural and engineering services for various projects with construction costs of less than $500,000 to repair State owned buildings that were damaged by Hurricanes Katrina and Rita. These projects will consist of repair work to buildings of all types and sizes and may include architectural, mechanical, electrical, civil, structural, environmental work, either separately or combined. Projects may also include new construction, restoration, remedial and other work. Upon expiration of the stated deadline Facility Planning & Control will prepare and maintain a file of all qualified applicants. When architectural and/or engineering services are needed for a project with a construction cost of less than $500,000, Facility Planning & Control will review the applications and determine the most appropriate firm based on the contract requirements. That firm will be offered the contract with the program and at the fee, contract design time and liquidated damages established by Facility Planning & Control. If the firm declines the offer, Facility Planning & Control will continue the selection process and that firm shall remain on the list. However, if a firm declines three consecutive offers it shall be removed from the list. Fees will be established by Facility Planning & Control for each project based on the standard fee formula and appropriate modifiers as stated in the Capital Improvements Procedure Manual for Design and Construction, 2004 edition. Contract design time including review time will be established by Facility Planning & Control for each project and may be aggressive. Liquidated damages will also be established in a daily amount that is in accordance with time sensitivity of the project. Consultants shall be selected by the designer subject to the approval of Facility Planning & Control. The designer shall prepare and submit all required drawings to Facility Planning and Control in AutoCAD Release 2000 or lower on 3.5" DS, HD diskettes or CD ROM. Files may be compressed (PKZIP) but if so, provide software to uncompress (PKUNZIP). Drawings shall follow the format specified in the "Instructions to Designers for AutoCAD Drawings Submittal. Applicants are advised that design time ends when the Documents are "complete, coordinated and ready for bid" as stated in to Article 3.3.1 4) of the Capital Improvements Projects Procedure Manual for Design and Construction. Documents will be considered to be "complete, coordinated and ready for bid" only if the advertisement for bid can be issued with no further corrections to the Documents. Design time will not necessarily end at the receipt of the initial Construction Documents Phase submittal by Facility Planning and Control. Any re-submittals required to complete the documents will be included in the design time. In addition to the statutory requirements, professional liability insurance with a limit of liability of at least $500,000 covering the work involved will be required in an amount specified in the following schedule. This will be required at the time the designer's contract is signed. Proof of coverage will be required at that time. Applicant firms should be familiar with the above stated requirements prior to application. The firm(s) selected for the project(s) will be required to sign the state's standard Contract Between Owner and Designer. When these projects are financed either partially or entirely with Bonds, the award of the contract is contingent upon the sale of bonds or the issuance of a line of credit by the State Bond Commission. The State shall incur no obligation to the designer until the Contract Between Owner and Designer is fully executed. When a firm accepts a project it will be expected to have all expertise necessary to provide all architectural and/or engineering services required by the Louisiana Capital Improvement Projects Procedure Manual for Design and Construction for the project. Unless indicated otherwise in the program, there will be no additional fee for consultants. SPECIALIZED REQUIREMENTS: This project has specialized requirements consisting of (1) Simultaneous construction of multiple large facilities; and (2) Phased Construction. EVALUATION CRITERIA - Interested firms will be required to have a design team with experience completing design-build construction of facilities up to five stories. The disciplines involved, but not limited to, are: Architectural, Civil, Structural, Mechanical, Electrical, Communications, DQC Manager, and Fire Protection. The Fire Protection Engineer must meet the requirements of UFC 3-600-01, Paragraph 1-6, Services and Qualifications of Fire Protection Engineers?. Communications designer must be a Registered Communications Distribution Designer (RCDD). The design team shall include at least one LEED Accredited Professional as recognized by U.S. Green Building Council. The design team must include a LEED Accredited Professional that has acted as team leader for a LEED certified building(s). The Warren County Board of Supervisors, Mississippi will accept sealed proposals from qualified firms for professional services required for application preparation and subsequent implementation of their Community Development Block Grant Public Facilities and Economic Development Disaster Program and/or any other program as designated for Disaster Relief by the Mississippi Development Authority. The County is seeking funding for the Community Development Block Grant Public Facilities and Economic Development Disaster Assistance and a Project Administrator, Project Architect, Project Engineer, and Project Attorney will be selected to oversee various Disaster projects. Information packages concerning the request for proposals are available at the Chancery Clerk's Office, 1009 Cherry Street, P. O. Box 351, Vicksburg, MS 39183 between the hours of 8 A.M. and 5 P.M., Monday through Friday. The services required for this project are: ADMINISTRATIVE: Assist the County and its agents in the application preparation and subsequent program administration including management accounting, required reports to HUD and the State, implementation of applicable State and Federal laws, regulations, and requirements, establishment and maintenance of filing system; and serve as liaison between the County and the State. Firms or individuals proposing to perform administrative services should submit a list of their qualifications, experience, and capacity for performance. ARCHITECT: Architectural design for construction of the proposed improvements including: all design surveys, preliminary design work, final design phase, preparation of plans and specifications, and construction inspection. Firms or individuals should submit a list of their experience, qualifications, and capacity for performance. ENGINEERING: The selected engineer shall complete engineering services in accordance with applicable state and federal laws, regulations and procedures. Activities will include insuring compliance with all aspects of project design and implementation. More specific services required for this project include: (a) Prepare plans and specifications for project. (b) Construct and distribute bid packets (insuring that all Federal and State requirements are met in contract preparation). (c) Assist in bid opening and prepare bid tabulation. (d) Provide resident project representative to insure contractors conform to plans and specifications. (e) Conduct work-in-progress inspections giving monthly reports to the County and approving any and all partial payment requests. (f) Review and approve all change order requests prior to submission to the Warren County Board of Supervisors or their designee for final approval. LEGAL: Consultation services as required for the implementation of the proposed improvements as outlined in the Disaster application in accordance with State and Federal laws and regulations. Legal services shall include but not be limited to consultation services, the evaluation of all contracts and land acquisition, etc. Offerors should submit a list of their experience, qualifications, capacity for performance along with an hourly rate charged for consultation services. Other services include examination of property records related to any easements which might be required. Proposals for this project will be rated according to the following criteria: 1. EXPERIENCE with similar Community Development Block Grant Projects; 2. QUALIFICATIONS, knowledge, and technical expertise in this and similar projects; 3. CAPACITY FOR PERFORMANCE to perform required services in a timely manner, given current workload and staff. All proposals will be rated on the following system to determine the best offeror: Description and maximum points: Qualifications - 40; Experience - 40; Capacity - 20; Total: 100 Points. Proposals will be reviewed by the selection committee using the above selection criteria. A written contract will be awarded to the firm whose proposal is within the competitive range and determined by the committee to be the most advantageous to the County, cost and other factors considered. The contract will include scope and extent of work and other essential requirements. The Warren County Board of Supervisors reserves the right to reject any and all proposals and to waive any irregularities or informalities in the proposal process. The State of Louisiana, Division of Administration Office of Community Development Disaster Recovery Unit hereby solicits proposals from qualified proposers to provide environmental clearance reviews and environmental consulting services in conjunction with portions of the Hurricane Katrina and Hurricane Rita disaster recovery programs. Services to be provided shall include, on a state-wide and/or site specific, as-needed basis, determining the level of and conducting environmental reviews for, the Community Development Block Grant Disaster Recovery Programs in accordance with 24 CFR Part 58, as well as coordination with oversight /regulatory agencies for receipt of necessary permits for compliance resolution. Task Assignment Title: The Bethesda Hospitals Emergency Preparedness Partnership consisting of the National Naval Medical Center, National Institutes of Health Clinical Center, and Suburban Hospital Healthcare System requires the purchase of (2) multi-functional portable medical shelters/field hospitals and support/storage trailers utilized to immediately and effectively manage large numbers of casualties or walking wounded during a mass casualty/disaster incident. B. Requirements: The following minimum detailed requirements/specifications for the Partnerships Portable Medical Shelters/Mobile Field Hospitals are indicated below: * Require (2) mobile medical shelters/field hospitals (one for the National Naval Medical Center and one for Suburban Hospital Healthcare System). The shelters must be portable, rapidly deployable and transportable to various locations within the facility campuses, or throughout the National Capital Region. * Minimum size of each shelter system must be at least 100 feet long by 22 feet wide to ensure sufficient space for at least 25 beds/stretchers, patient care staff, administrative support staff and portable medical equipment and supplies. Maximum size of each shelter system should be no more than 120 feet long by 24 feet wide. To accommodate storage and transportability, require a series of interconnected/attachable structures, NO MORE THAN THREE ATTACHABLE STRUCTURES (e.g. one main large patient care shelter and two smaller shelters ? all interconnected to obtain a minimum total size of at least 100 feet long by 22 feet wide). * Minimum Interior Height of Shelters (e.g. ground level to center of ceiling) must be a minimum of 15 feet. * Require pack-out dimension of each shelter to be no more than 4 foot wide by 6 foot long by 4 foot high to accommodate storage requirements. * Require shelters to be inflatable (e.g., series of durable inflatable individual columns) versus a hard framed structure. Inflatable columns must have a durable protective outer cover with a replacement tube/inner lining. * Require a power/vacuum pump (one for each shelter) with adequate horsepower to inflate columns. Specific type of pump and required horsepower/amps to be proposed/determined by selected vendor. * Require a portable, roll out, attachable flooring system which covers entire shelter area and can be easily installed with minimal manpower (less than 5 personnel) - one complete flooring system for each shelter. The portable flooring system must be durable enough to accommodate the weight of portable equipment, rolling stretchers and personnel moving throughout the shelter. * Shelters must be wind/weather resistant (must sustain wind gusts up to 80MPH and snow loads of up to 6-8 inches). Material must be certified to the highest national fire code standards for temporary structures. * The (2) shelter systems must be equipped with electrical outlets (a minimum of 12 interior duplex electrical outlet boxes of at least 115 volts outlets must be 3 prong. Exact type of outlet, location and attachment/mounting system will need to be determined/proposed by selected vendor. * The (2) shelter systems must be equipped with electrical lighting (a minimum of 17 high intensity 54 watt overhead fluorescent lights 2 bulbs per light). Require all lighting be interconnected to each other. Specific type of lighting and process for mounting/interconnecting throughout the shelters will need to be determined/proposed by selected vendor. * The (2) shelter systems must be equipped with a Diesel Generator, one for each shelter (a minimum of 105KW) to support the HVAC system and power to operate portable medical equipment (e.g. IV pumps, ventilators, monitors, etc.). Each generator must be equipped with a sound level package (maximum of 67 decibels) and an exhaust system. Each generator must have a minimum of (1) 220 Volt outlet. Generator must be custom mounted to a portable towing trailer. Specific type of generator, mounting design, etc to meet required specifications noted above to be determined/proposed by selected vendor. * The (2) shelter systems must be equipped with an HVAC system, one for each shelter (must be at least 15 ton capacity) to regulate temperature during period of extreme warmth/coldness. (Each HVAC System must be equipped with a minimum of 2 foot wide by 25 foot long flexible ducting). HVAC system must be custom mounted to a portable towing trailer. Specific size/exact features and mounting design of HVAC system to be proposed/determined by vendor. * The (2) shelter systems must also be equipped with a Portable Hepa-Filtration System, one for each shelter (a minimum of 3 Ton ECU). Each Hepa-Filtration system must be equipped with a minimum of 2 foot wide by 25 foot long flexible ducting. Specific type/exact features of portable Hepa-Filtration system will need to be determined/proposed by vendor. * The (2) shelter systems must be equipped with windows and doors (e.g. at least (1) door entry at each end of the shelters and several windows along each side of shelters). * The (2) shelters must also be equipped with (2) custom designed storage trailers (one for each shelter) for storage and transportability, which meets highway safety standards. Storage trailers need to be equipped with pull down ramp, lighting and locking mechanism. Specific size of each trailer will be dependent on overall pack-out size of each shelter system (Vendor to propose configuration). * Require (2) transport carts/dollies (one for each shelter system) to transport shelters from storage trailer to set-up/deployment location (Size of transport cart/dollie dependent upon overall pack-out dimension of shelter (configuration to be proposed by vendor). * The (2) shelter systems must also be equipped with (2) custom designed support trailers (one for each shelter) for Generator and HVAC system, which meets highway safety standards. Specific size of each trailer will be dependent upon overall size of generator/HVAC System (configuration to be proposed by Vendor). * The (2) shelter systems must have a custom designed shelter anchoring system to secure shelters to ground. Shelters will be portable and will be set up in various locations on various surfaces and will need to be secured. * The shelters must be rapidly deployable/set-up in one hour or less with 5 or less personnel. The shelters must be light weight and not rely on heavy equipment/machinery for off-load, transport and set-up. C. Warranty: Require a minimum of (2) year warranty from date of delivery. D. Training/Customer Support: a. Require at least 8 hours of initial training upon delivery of shelters be provided by vendor regarding set-up and operation of shelters. b. Require an additional 16 hours of training be made available during the 1st year following delivery for set-up and operation of shelters by vendor. c. Require vendor provide training on-site at the National Naval Medical Center, Bethesda, Maryland. E. S. Coast Guard Maintenance and Logistics Command Atlantic (MLCLANT) is procuring services under Solicitation HSCG84-07-Q-AA8183 which will be available on or about September 21, 2006 via FedBizOps. In an event of severe weather, during the hurricane season which runs from May 1 to November 30, members of the Joint Forces Offices (JFO) of the U. S. Coast Guard Sector New Orleans would need a safe location that is away from current hurricane and potential future hurricane hazards and dangers to lives. The facility would need to accommodate certain logistical needs in order to properly lead U. S. Coast Guard post hurricane operations. The U. S. Coast Guard requires a contracting vehicle to be in place that will allow us to have the "first right" to use the facility in an event of severe weather affecting the Gulf shore resulting in evacuation of New Orleans, LA. At least 96 hours prior to landfall of anything equal or greater than tropical force winds, an advance team will be deployed to this location to set up for JFO stand up. At 48 hours prior to landfall, a decision will be made to expand the operation and more personnel will be arriving. If a similar storm to Hurricane Katrina hit, this will require that USCG Sector New Orleans JFO members to stay for an undetermined period of time, due to the inability to return to normal operations in the New Orleans area. It is anticipated that the minimum period of time would be 3 days with a maximum of 42 days per occurrence. The requirements for the facility are as follows: The JFO location needs to be located in Baton Rouge, Louisiana in order for the team to work out of the Integrated Command Post located in Baton Rouge. A total of 25 hotel rooms will be needed. Parking will be needed for up to 50 vehicles. The NAICS code is 721110. The Small Business Size Standard is $6.5 million. The period of performance is anticipated for five-one year ordering periods commencing October 2006. All vendors will be responsible for obtaining the solicitation and any subsequent amendments from the above site.
