8/11/2008 - The Council plans to approve the following: Resolution No. 339-08(F) providing appropriations for the receipt of Federal EmergencyManagement Agency (FEMA) funds for the Banyan Street Parking Garage Repairs.A RESOLUTION OF THE CITY COMMISSION OF THE CITY OF WEST PALMBEACH, FLORIDA, AUTHORIZING THE APPROPRIATION OR TRANSFER OF CITYFUNDS IN FISCAL YEAR 2007/2008 IN ACCORDANCE WITH SECTION 4.03 OF THECITY CHARTER OF THE CITY OF WEST PALM BEACH, FLORIDA, FOR THEPURPOSE OF AMENDING THE SELF INSURANCE FUND BUDGET TO PROVIDEAPPROPRIATIONS FOR THE RECEIPT OF FEDERAL EMERGENCYMANAGEMENT AGENCY (FEMA) FUNDS FOR REPAIRS TO THE BANYAN STREETPARKING GARAGE; PROVIDING AN EFFECTIVE DATE; AND FOR OTHERPURPOSES. Agenda Cover Memorandum No.: 16972Staff Recommended Motion:Approve Resolution No. 339-08(F).Background:The Banyan Boulevard Parking Structure was built in the late 1970s and consists of a four (4)level, 2-bay double thread structure. The structure has a footprint of approximately 123 feet by304 feet. The existing structural system includes a conventionally reinforced concrete cast-in-placeconcrete slab and precast single tee beams and columns. The precast beams and columns are at 18foot centers and are connected, using a partial moment connection at each end of the beams, tocreate a wind frame on each grid line of the structure. These frames resist lateral loads and transferthese loads to the foundations. The structure has an expansion joint near its center and is supportedon piles.The structure experienced structural and nonstructural damage due to Hurricane Wilma. Tim Haas,consultants, noted in their report that most of the damage observed inside the structure can beattributed to excessive lateral movement and rotation of the structure during the hurricane. Someof the typical damage inside the structure included spalled and/or cracked concrete and failedsealants, most of which do not reduce the structural integrity of the garage. Damage adjacent tomany of the beam to column wind connections in the center of the structure was of concern to theinspectors. On the lower floors this damage did not appear to have reduced the structural capacityof the connection and it was recommended to install new sealants and concrete sealer above theseconnections to protect them from water intrusion and possible corrosion damage, however on theupper floors the damage was more severe and resulted in the failure of some of the windconnections. This reduced the capacity of the connection and it was recommended that theseconnection be repaired.Tim Haas inspectors met with FEMA representatives to discuss their findings and acknowledgedthat the structure had experienced some damage due to Hurricane Wilma. FEMA indicated thattheir program would repair items in kind and upgrade damaged items if it can be shown that theupgrade will reduce the possibility of future hurricane damage. Slider Engineering, subconsultantsof Civil Design, Inc were hired to prepare drawings for both structural andnonstructural repairs as well as improvements in the design. This repair will bring the structureback to the capacity it was originally designed for.The Engineers cost estimate for the this work totalled $529,182.00. Of which $243,107.00 werefor the repairs to the columns, corbels, cracks, surfaces, masonry and spalls. The City wasapproved, by FEMA, to complete Hazard Mitigation improvements to the column/tee shearconnections and the facade bracing, estimated at $286,075.00. These improvements were limitedto the top floor only and would meet the requirements of the Building Code.Fiscal Note:Resolution No. 339-08(F) provides appropriations of $318,002 from FEMA funds for this project.In addition, $211,180 is currently available in the budget, for a total project cost of $529,182.

SPECIAL ALERT: HURRICANE KATRINA RELIEF EFFORT

 

By virtue of and in conformity with the provisions of Act 70 of the 1946 Regular Session of the Louisiana Revised Statutes of 1950, as amended, and other applicable laws, sealed bids will be received by the Plaquemines Parish Government at the Plaquemines Parish Engineering and Public Works Department, 102 Avenue G, Belle Chasse, Louisiana, before 10:00 a.m., CDT, Tuesday, the 26th day of August, 2008, at which time and date such bids will be publicly opened and read for the REPLACEMENT OF THE RECREATION/SIGN/PROWM BUILDING, 27279 Highway 23, Port Sulphur, Louisiana 70083. Plaquemines Parish Project No. 06-06-03. PROJECT DESCRIPTION: The Recreation/Sign/PROWM Building is a new building to be constructed on Highway 23 in Port Sulphur, Louisiana to replace the previous building which was destroyed at this site by Hurricane Katrina. The new building is a two story approximately 9,900 square foot building on a pile supported concrete slab and foundation with concrete columns at the first level supporting a poured in place second floor reinforced concrete slab. The sloped roof is to be fabricated of cold formed metal roof trusses which are supported by structural steel tube columns which occur at the second level. The building has interior partitions of concrete masonry units and gypsum board over metal studs. The exterior skin of the building is insulated pre-finished metal wall panels with a sloped pre-finished standing seam metal roof. The interior of the building consists of two service bays at the first level which serve as the Vehicle Maintenance Garage for tractor repair and maintenance. The building also consists of interior office space, public lobby areas, elevator, enclosed fire stairs, restroom facilities, equipment storage and miscellaneous other spaces. The new Recreation/Sign/PROWM Building is to be conditioned with split DX HVAC systems. The exterior HVAC condensing units are supported on an elevated structural steel mechanical equipment pad. The sitework is limited to new concrete driveway areas and limited concrete parking areas. All in accordance with specifications on file at the Plaquemines Parish Engineering and Public Works Department, 102 Avenue G, Belle Chasse, Louisiana, which may be obtained by any bonafide Louisiana Licensed Contractor at the office of Yeates and Yeates Architects, L.L.C., 929 South Peters Street, New Orleans, Louisiana 70130 at a cost of Two Hundred Dollars and no cents ($200.00) for each set of documents. Deposit on the first two sets are fully refundable to all bonafide prime Bidders upon return of the documents, in good condition, no later than ten (10) days after receipt of bids. Fifty percent (50%) of the deposit of all other sets of documents will be refunded upon return of documents as stated above. SEALED BIDS will be received for the Plaquemines Parish Government, at the Plaquemines Parish Engineering and Public Works Department, 102 Avenue G, Belle Chasse, Louisiana 70037 until 10:00 A.M., with bid opening at 10:00 A.M., on August 26, 2008 for the Hurricane Katrina Damage Repairs Point A La Hache Boat Harbor, Phase II. PPG Project # 06-01-08. Project Scope - The general scope of the project titled Hurricane Katrina Damage Repairs Pointe A La Hache Boat Harbor, Phase II includes reconstruction of the Pointe A La Hache Boat Harbor, including, but not limited to, dredging of the harbor, repair and / or replacement of damaged timber deck boards, timber stringers, timber cross bracing, pier approach grading and timbers, water lines, hose bibs, fire hose and fire reels, sewer treatments facility, and oil containment tank; removing and replacing existing mooring and guide piles; covered boat slip structure; and reconstruction of the harbor master’s office/guard building; as shown on the drawings for the Plaquemines Parish Government in Pointe A La Hache, Louisiana. By virtue of and in conformity with the provisions of Act 70 of the 1946 Regular Session of the Louisiana Revised Statutes of 1950, as amended, and other applicable laws, sealed bids will be received by Plaquemines Parish Engineering at the Plaquemines Parish Engineering and Public Works Department, 102 Avenue G, Belle Chasse, Louisiana, before 10:00 am, CDT, Tuesday, the 2nd day of September 2008, at which time and date such bids will be publicly opened and read for the EAST POINTE A LA HACHE WATER TREATMENT PLANT HURRICANE REPAIRS PHASE II, Project No. 06-01-04. The project consists of the facility-wide repair of the East Pointe a la Hache WTP due to flooding and wind damage caused by Hurricanes Katrina and Rita. The facility includes water clarifying, filtering, and storage tanks, an office control building, and a raw water intake structure. Major repair components include installation of buried piping with levee and road crossings, tank repair and/or replacement, construction of a sub-surface, steel re-enforced concrete storage tank, pump, piping, and valve replacement, masonry repair, installation of miscellaneous concrete foundations, structural steel fabrication, lighting, electrical conduit, feeder, and control panel replacement, preparation and painting of building interiors and exteriors, tanks, piping, and miscellaneous steel. The filter/control building is a 2900 square foot two-story, CMU structure. The new raw water building on the Mississippi River batture is a 90 square foot steel framed structure, skinned with pre-finished metal panels. The contracted work shall provide for furnishing all labor, material, supervision, transportation, demolition, disposal, testing, and performing all work as indicated in the drawings and specifications. The contractor must be properly licensed and insured to perform all functions necessary to provide an operable facility at the conclusion of construction. Copies of the Contract Documents, consisting of the drawings and specifications may be examined without charge in the office of Infinity Engineering Consultants, LLC, 2626 Canal Street, New Orleans, LA 70119. SPECIAL ALERT: HURRICANE KATRINA RELIEF EFFORT

 

INVITATION TO BID Bids for the HURRICANE KATRINA DAMAGED ROADS PROJECT addressed to the Hancock County Board of Supervisors will be received in the Hancock County Board of Supervisor's office, at 3068 Longfellow Drive, Building 3, Suite A, until 11:00 A.M. local time on the 15th day of September, 2008. Any bids received after the time and date specified will not be considered and will be returned unopened. This project involves repairing 120 various roadways throughout Hancock County. Repair work includes asphalt base repair, asphalt leveling and asphalt overlay. There will also be minimal amounts of traffic striping and removal and replacement of concrete curb. Contract Documents may be examined at the Design Engineer's Office located at 3036 Longfellow Drive, Bay St. Louis, Mississippi. A copy of the documents with half scale drawings may be obtained at the office of Compton Engineering, P.A., Post Office Box 2795, Bay St. Louis, Mississippi 39521 upon payment of $150.00 for half scale and an additional $100.00 for full size, which includes shipping and handling. Return of the documents is not required, and the amount paid for the documents is non-refundable. For information regarding this project, contact Compton Engineering's office at (228) 467-2770 in Bay St. Louis, Mississippi. Each proposal must be submitted on the prescribed form and accompanied by a certified check or bid bond executed on the prescribed form, payable to the Hancock County Board of Supervisors in an amount not less than 5 percent of the amount bid. The successful bidder will be required to furnish the necessary additional bond(s) for the faithful performance of the Contract, as prescribed in the Contract Documents. All performance bond, payment bond and certificates of insurance shall be either executed by or countersigned by a licensed resident agent of the surety or insurance company having his place of business in the State of Mississippi and in all ways complying with the insurance laws of the State of Mississippi. Further, the said surety or insurance company shall be duly licensed and qualified to do business in the State of Mississippi. In order to perform work for projects over $50,000, the successful bidder shall, as applicable, hold or obtain such Contractor's and business licenses as required by state statutes including certification of responsibility required by Section 31-3-21, Mississippi Code of 1972, as amended. Bidder's certification number or a statement that the Bid inside is less than $50,000 shall appear on the outside or exterior of the envelope or container of such bids, failing which said bid shall not be opened or considered. Changes for this bid shall not be made by notations on the exterior of the container of the bid. SPECIAL ALERT: HURRICANE KATRINA AND RITA RELIEF EFFORT

 

By virtue of and in conformity with the provisions of Act 70 of the 1946 Regular Session of the Louisiana Revised Statutes of 1950, as amended, and other applicable laws, sealed bids will be received by Plaquemines Parish Engineering at the Plaquemines Parish Engineering and Public Works Department, 102 Avenue G, Belle Chasse, Louisiana, before 10:00 am, CDT, Tuesday, the 26th day of August, 2008, at which time and date such bids will be publicly opened and read for the DALCOUR WATER TREATMENT PLANT HURRICANE REPAIRS, Project No. 06-01-05. The project consists of the facility-wide repair of the Dalcour WTP due to flooding and wind damage caused by Hurricanes Katrina and Rita. The facility includes water clarifying, filtering, and storage tanks, an office control building, and a raw water intake structure. Major repair components include pump, piping, and valve replacement, masonry repair, structural steel fabrication, lighting, electrical conduit, feeder, and control panel replacement, preparation and painting of building interiors and exteriors, tanks, piping, and miscellaneous steel. The filter/control building is a 2900 square foot two-story, CMU structure. The raw water building on the Mississippi River batture is a 150 square foot steel framed structure, to be re-skinned with pre-finished metal panels. The contracted work shall provide for furnishing all labor, material, supervision, transportation, demolition, disposal, testing, and performing all work as indicated in the drawings and specifications. The contractor must be properly licensed and insured to perform all functions necessary to provide an operable facility at the conclusion of construction. Copies of the Contract Documents, consisting of the drawings and specifications may be examined without charge in the office of Infinity Engineering Consultants, LLC, 2626 Canal Street, New Orleans, LA 70119. Plans, specifications and bid forms may be obtained from Infinity Engineering Consultants, LLC, 2626 Canal Street, New Orleans, LA 70119, by prospective bidders upon deposit of $100.00 for the first set obtained. Deposit will be refunded to bona-fide prime contract bidders upon return of plans, specifications, and addenda in good condition within ten (10) calendar days after date of opening bids. Prime contractors may obtain additional sets and subcontractors, suppliers, etc. may obtain sets upon the payment of $100.00 per set, with $100.00 per set, refundable upon return of plans, specifications, and addenda in good condition within ten (10) calendar days after date of opening bids. Bidders shall be licensed pursuant to appropriate laws, rules and regulations of the State of Louisiana and the Parish of Plaquemines and shall submit evidence that they hold license of proper classifications in full force and effect at the time the plans are obtained. License number shall be shown on the outside of the sealed bid. Each bidder must deposit with his bid security in the form of cash, certified check of the bidder or a Bid bond in the amount of five (5%) percent of the bid. The City of Long Beach will accept sealed proposals from qualified individuals and firms for Project Management Services for municipal projects with multiple funding sources that include the Mississippi Development Authority with Katrina Supplemental CDBG Program funds. The CITY has the option of using the selected firm for PM services related to FEMA/MEMA funding, city funding and/ or CDBG funding. All services will be provided in accordance with applicable state and federal law and regulations governing the funding source which includes but is not limited to the CDBG program. An "Information to Offerors Packet" concerning this request for qualifications is available at City Hall, 645 Klondyke Road, Long Beach, Mississippi 39560 between the hours of 8:00 A.M. and 4:00 P.M., Monday through Friday. Qualifications for Project Management Services will be reviewed and rated in accordance with the following rating factors/criteria: 1. Qualifications Information reflecting the background, professional and technical expertise to carry out the work. 2. Experience Information regarding experience of the firm and of the individuals to be assigned to provide the services. 3. Capacity for Performance (Work Force) Timely completion of the work is essential, to that end, each proposal must clearly identify the number and title of staff to be assigned to complete the work. All proposals will be rated based on the following rating system, which will be used to determine the best/ acceptable offeror:Qualifications 40 points Experience 40 pointsCapacity for Performance 20 points Note: The City of Long Beach reserves the right to select one firm to provide project management services or to select a number of firms to provide the services. Proposals will be reviewed by a Consultant Selection Committee using the above selection criteria.
