STRUCTURAL ENGINEERING SERVICES Project Title: HSIR SEISMIC RETROFIT STUDY
Project No.: M8417 The University of California, San Francisco (UCSF) is in
process of selecting a qualified structural engineering firm for the referenced
project. Those wishing to be considered must respond to the requirements of the

UCSF is embarking on a Seismic Assessment Program to address UCSF buildings
containing critical research assets (or other critical operations) that need to
remain structurally sound and functional after a major earthquake. The program
includes buildings at Parnassus and other UCSF locations. This project will
begin with the two Health Sciences Towers and other buildings may be added
later. The goal of the proposed work is to determine the least costly means for
achieving safe occupancy in the buildings after a Design Level Seismic event,
particularly considering advanced analysis techniques and new construction
technologies. Final selection and appointment is contingent upon funding and
budget approval. Every effort will be made to ensure that all persons,

Orleans Levee District is accepting Statements of Qualification for the damage
repair design and construction management of both the Mardi-Gras Fountain,
located on Lakeshore Drive, and the historic Four Winds Fountain, located on
the Lakefront Airport in New Orleans. Both fountains were damaged during
Hurricane Katrina. Architects and Engineers will be evaluated for
qualifications, including experience, key personnel, similar projects,
references, and current work availability and schedule. A short list of
qualified firms will be developed and further interviews and/or presentations may

PROJECT DESCRIPTION
A. The
work consists of repairing and replacing damaged public housing facilities
resulting from Tropical Depression IKE on September 14, 2008. The extent of the
work is indicated in the contract documents. The work consists, but is not
limited to, the repairing or replacing the following: roofing, exterior siding,
soffits, gutters, downspouts, metal fascia and trim, wood trim and rough
framing, fiberglass batt insulation, metal flashing, masonry replacements,
painting, suspended acoustical ceilings, and additional work as listed in the
Project Manual and Drawings. Selective demolition, renovations, and cutting and
patching shall be as required for new construction as part of this project.
1.
Project Locations: As shown by street names on Site Plans of projects of the
Housing Authority, City of Jeffersonville, Indiana.

Jeffersonville Housing Authority, 206 Eastern Boulevard, Jeffersonville,

B.
Contract Documents, dated January 29, 2009, were prepared for the Project by
Michell Timperman Ritz Architects, 319 Bank Street, New Albany, Indiana 47150.
C. The work will be installed under the
single prime contract.
D. The
work consists of labor and materials, bonds, equipment, supervision, and
insurance required to install new work as indicated on Contract Documents.
1.3 CONSTRUCTION USE OF PREMISES
A. Use of
the Site: Limit use of the premises to work in areas indicated. Confine
operations to areas within contract limits indicated. Do not disturb portions
of the site beyond the areas in which the Work is indicated.

1.0 GENERAL
1.1 Sealed Bids will be received by The
Housing Authority of the City of Jeffersonville, Indiana, until 2:00 P.M.,
(Local Time) on Tuesday, March 31, 2009, in the Board Room of The Housing
Authority of the City of Jeffersonville, Indiana, 206 Eastern Boulevard,

1.2 A Pre-Bid Conference will be held at the
Board Room of The Housing Authority of the City of Jeffersonville, Indiana, 206
Eastern Boulevard, Jeffersonville, Indiana at 2:00 P.M., (Local Time), on
Tuesday, March 17, 2009. All bidders, including subcontractors, are encouraged
to attend. Prior to the Pre-Bid Conference, bidders should obtain the bid
documents from the Architect's office.
The
Pre-Bid Conference is for the purpose of permitting Bidders and their
subcontractors to inspect the areas of the proposed work and to receive the
Architects answers or interpretations to contractor's questions prior to
issuance of an Addendum.
1.3 A Lump Sum Bid will be received from
Contractors for all work as shown on the drawings and hereinafter specified and
as set forth on the Contractor's Bid Form.
1.4 Bids will be opened and publicly read
aloud. Submit bids in triplicate, (3) copies.

1.6 Contractor's may obtain sets of drawings
and specifications from the Office of Michell Timperman Ritz Architects, 319
Bank Street, New Albany, Indiana. A $100.00 deposit check will be required for
construction documents. The deposit will be refunded if the set is returned in
good condition within 10 (ten) days after the opening of bids, unless
hereinafter stated otherwise. IF THE CONTRACTOR DOES NOT SUBMIT A BID AND/OR
DOES NOT RETURN THE CONSTRUCTION DOCUMENTS WITHIN THE TIME HEREINBEFORE STATED,
THE COMPLETE DEPOSIT WILL BE AWARDED TO THE OWNER TO COVER REPRODUCTION OF THE
CONTRACT DOCUMENTS. Checks will be cashed upon receipt by the Architect.
1.7 The work for which proposals are
requested includes all bonds, insurance, labor, materials, equipment, and items
of materials and services required to provide the proper and complete
construction of this project. The work shall be constructed in accordance with
this Invitation To Bid, Drawings and Project Manual as prepared for this
project by Michell Timperman Ritz Architects.
1.8 Bids and Bid Guarantees shall be placed
in sealed envelopes and addressed to:

Sealed
envelope shall list the name and address of the Bidder.
1.9 Bid shall be executed on Indiana State
Board of Accounts Form #96 (Revised 2005) with Non-Collusion Affidavit, sworn
to as required by the Statutes of Indiana, for all bids of $5,000.00 or more.
All other data required by Bid Form 96, Part II and Part III must be submitted
as required by Bid Form No. 96.
1.10 Each bid package proposal shall be
accompanied by certified check or bank draft, payable to The Housing Authority,
City of Jeffersonville, Indiana, U.S. Government Bonds, or a satisfactory bid
bond executed by the bidder of an acceptable surety in an amount equal to five
percent (5%) of bids.
1.11 The successful bidder will be required to
furnish and pay for satisfactory 100% Performance Bond and Labor and Material
Payment Bond or Bonds, before starting any work or placing materials on the
job. The bonds shall be received with 10 (10) days of the Notice to Proceed
date.
1.12 All prices quoted, are to be free of all
State Sales Tax, and upon request of supplier, a tax exemption certificate will
be executed by the Owner to the successful bidder to cover all items subject to
this tax.
1.13 Attention is called to the provisions for
equal employment opportunity, and payment of not less than the minimum salaries
and wages as set forth in the specifications must be paid on this project.
1.14 Attention is called to the fact that the
Contractor must take affirmative action to insure that applicants are employed,
and that employees are treated during employment, without regard to their race,
color, religion, sex or national origin.
1.15 Contractor's are required to comply with
the Provisions of Section 3 as set forth on 24 Part CFR B5 as follows:
A. A good faith effort to utilize Section
3 area residents as trainees and employees.
B. A good faith effort to award contracts
for work in connection with the Project on Section 3 business concerns.
1.16 Attention is called to a request that the
Contractor shall make a best efforts goal to achieve a state goal of 10%
minority and/or women business enterprise participation in this project.
1.17 All Bidders are required to submit with each
copy of their bids the following:
1. Contractor's Bid for Public Works, Form
96, as prescribed by the State Board of Accounts of Indiana, complete with
Section II, Plan and Equipment Questionnaire; Section III, Contractor's
Financial Statement; and Section IV, Oath and Affirmation.
2. HUD Form 5369A, Representatives
Certification and Other Statement of Bidders.
3. Non-Collusion Affidavit of Prime Bidder
(on the State Board of Accounts Form 96).
4. Bid Bond.
5.
Previous Participation Certificate Form 2530 (if the Contract is $50,000 or
more).
1.18 Should any Bidder withdraw his bid or bids
within forty five (45) days after the time of Bid Opening or if issued a Letter
of Intent, withdraw his bid or bids after the issuance of such Letter of
Intent, or fail to execute a Form of Agreement if presented, the owners may
declare the Bid Bonds forfeited to the Owners as liquidated damages.
1.19 Bidders not issued a Letter of Intent will
have their Bid Guarantees returned within 10 days after the issuance of a Letter
of Intent or Contract to the successful contractor.
1.20 Bids may be held by the The Housing
Authority, City of Jeffersonville for a period not to exceed forty five (45)
days from the date of the opening of Bids for the purpose of reviewing the Bids
and investigating the qualifications of Bidders prior to awarding the Contract.
Bidders shall state in HUD Form 5369A, Article 6(d) if a longer acceptance
period is offered.
1.21 The Jeffersonville Housing Authority
reserves the right to reject any or all bids or to waive any informalities in
the bidding

Engineering Contractors holding an ‘A’ license and General Building Contractors
holding a ‘B’ license are reminded that due to the Hawaii Supreme Court’s
January 28, 2002 decision in Okada Trucking Co., Ltd. v. Board of Water Supply,
et al., 97 Haw. 450 (2002), they are prohibited from undertaking any work,
solely or as part of a larger project, which would require the General
Contractor to act as a specialty Contractor in any area in which the General
Contractor has no license.

are solely responsible to review the project requirements, determine the
appropriate licenses required, and ensure that they possess and that the
Subcontractor(s) listed in their SOLICITATION, OFFER AND CONTRACT FORM possess
the necessary specialty licenses to perform the work for this project.

The project (referred to as East Marginal Way Grade Separation) provides for
the construction of a new vehicular overpass structure (to separate vehicular
traffic from rail traffic) located south of S. Spokane Street (lower Spokane
Street) and East Marginal Way intersection in Seattle, Washington. This
structure, located in a major regional highway and rail corridor, is south of
the Spokane Street, west of the Alaskan Way Viaduct (SR 99), and east of the
Duwamish Waterway. The project also includes minor improvements at the existing
street connections, realignment of a portion of South Spokane Surface Street to
pass under the grade separated roadway, and construction of a new at-grade
roadway for local access circulation. The Work also includes temporary traffic
control, erosion control, removal of pavement, grading and surfacing of
non-roadway areas, drainage, drainage vault, retaining walls, earthquake
drains, paving, channelization, signal modifications, illumination, signing,
and other required work. The traffic controller cabinet for the project and a
gate valve for a new hydrant have been procured by the Port of Seattle through
the Seattle Department of Transportation and Seattle Public Utilities. The
Engineer’s estimate range for this project is $14,000000 - $15,000000. Each bid
must be accompanied by a cashier’s check, money order, or surety bid bond, in
an amount of not less than five percent (5%) of the total bid, made payable to
Port of Seattle.
Performance
and Payment bonds will be required with the Contract. Time limit for

Sources Sought notice to make appropriate acquisition decisions regarding the
possible procurement of armoring/erosion control products and installation for
improving Louisianas Hurricane Storm Damage Risk Reduction System (HSDRRS). The
U.S. Army Corps of Engineers, Hurricane Protection Office solicits a Capability
Statement from all interested parties. All projects are located in the Southern
Louisiana parishes of Jefferson, St. Charles, Orleans and St. Bernard.
It is
anticipated that several types of materials will be used to armor the levees,
including but not limited to: reinforced vegetated systems, geosynthetics, turf
reinforcement mats and articulated concrete blocks. The Government is seeking
industry feedback on innovative solutions to supply and install armor on the
protected side of levees protecting against erosion.
The
current estimated placement of materials is as follows: Installation of
reinforced vegetated systems/turf reinforcement mats: approximately 24 million
square feet over 50 non-continuous miles. Sections have an average length of
14,000 feet and will include transition points e.g., roads, train tracks as
well as utilities and other obstructions. The estimated magnitude of the
project is $90M contemplated for firm fixed-price multiple awards. If an
Indefinite Delivery, Indefinite Quantity vehicle is contemplated, task orders
are anticipated at approximately $10M to $20M per task order. Reinforced
vegetated systems / turf reinforced mats must capable of withstanding a minimum
flow of 10 feet per second, with higher standards up to 17 feet per second
incorporated within some areas. Systems must be permanent, have UV protection
of 90% at 6,000 hours, and withstand light vehicle traffic and periodic mowing.
Installation
of articulated concrete block systems: approximately 18 million square feet
over approximately 35 non-continuous miles. Sections have an average length of
12,000 feet and will include transition points e.g., roads, train tracks, etc,
as well as utilities and other obstructions. The estimated magnitude of the
project is $260M contemplated for firm fixed-price multiple awards. If an
Indefinite Delivery, Indefinite Quantity vehicle is contemplated, task orders
are anticipated at approximately $10M to $50M per task order. Articulated
concrete blocks must meet a minimum flow of 15 feet per second with higher
standards incorporated within some areas.
The
estimated duration of installation is July 2010 with completion in Fall/Winter
2011.
All interested
parties are invited to submit a Capability Statement consisting of appropriate
documentation, literature and brochures providing responses addressing the
following:
(1) Firm
name, address, e-mail, telephone and fax number.
(2) If
submitting as a small business, provide identification of the company as a
Small Business with proper certification; 8(a), Hub Zone, Service Disabled
Veteran or Woman-Owned.
(3)
Address your capability to secure bonding for the projects described above in
the range of: $25 million to $300 million.
(4) Past
Experience: Experience installing erosion control products and specify the type
of product(s) used. Provide examples of projects completed including
installation rates, schedules and project size, ramp up / mobilization time,
equipment required, work site preparation required and list of references.
(5) Local
Market Knowledge: Describe your experience with armoring/erosion control
projects in the New Orleans area or within a 500 mile radius. Describe your
knowledge of and experience with material markets, subcontractors and the
general labor market.
(6)
Innovative Techniques, Methods, and Materials: Describe any proposed innovative
techniques, methods, and/or materials (not described herein) for armoring the
protected side of the levees against erosion.
(7)
Management Plan: Provide a brief plan of how you would accomplish the
installation of the quantity of products in the time frame allowed. In the
plan, address the following: (a) Material supply; (b) Supply strategy; (c) Weather
impacts; (d) Access issues; (e) Crew availability and required skill levels;
(f) Production rate (e.g., square foot installed per daydefine the hours in a
typical work day and composition of your work crew; (g) Project assumptions and
issues that impact pricing; (h) Estimated cost ranges per installed square
foot.
(8) List
of existing teaming agreements in place with other businesses that would be
needed to complete the effort.
Submission
of a Capabilities Statement is not a prerequisite to any potential future
offerings but participation will assist the Corps in making appropriate

Army Corps of Engineers (USACE), Hurricane Protection Office intends to issue a
solicitation for preconstruction services and construction option(s) for Levee
Improvements to the New Orleans East Back Levee, Reach LPV 111 in Orleans
Parish, LA. The solicitation will be a Request for Proposal for a Firm
Fixed-Price Base with a Fixed Price Incentive Option requirement in accordance
with FAR SubPart 16.403 utilizing Early Contractor Involvement (ECI) also known
as Construction Manager at Risk. The project is critical to the Hurricane Storm
Damage Risk Reduction System and must be completed by June 1, 2011.
This
requirement is an essential component of the New Orleans Hurricane Protection
System in Orleans Parish. Reach LPV 111 is an earthen levee which is adjacent
to the north side of the GIWW starting at the eastern terminus of the concrete
I-Wall just east of Michoud Canal at approximate Sta. 1342+74 running east to
Sta. 1105+50. From there, the levee section turns north and terminates at Sta.
1061+13 at the south wall of the CSX Railroad Gate, a total length of
approximately 28,161 feet or 5.3 miles. Included therein is a T-Wall from
approximate Sta. 1242+04 to Sta. 1235+51 with three 72 inch discharge pipes
from Pump Station No. 15 that pass through the stem of this structure. New
T-Walls were recently constructed at each end of the pump station. Also
included is the construction of a new concrete T-wall near the west end of LPV
111.01, as well as the construction of the tie in structure that will provide
permanent protection at the tie-in point of the IHNC surge wall. This option
includes approximately 1000 of new T-wall bringing the new protection elevation
to EL 32.0. The approximate stations of the new T-wall are from Sta. 1342+74 to
sta. 1352+74, with the tie-in structure extending approximately 100 feet past
to sta. 1353+74.
The
existing earthen levee sections are at El. 19.5 and were restored to this elevation
following extensive degradation from Hurricane Katrina. This project involves
raising the existing earthen levee to elevation varying from El. 28.0 to El.
31.5, as well as constructing new T-wall to a new protection elevation of EL
32.0. In order to accomplish the embankment construction while keeping the
footprint of the levee within the existing right of way, the foundation will
have to be strengthened by a Deep Soil Mixing (DSM) method. The Contractor will
need to closely coordinate with the AE designer of both the levee section and
the T-Wall section. The Contractor will also need to coordinate with the design
and construction of improvements to the CSX Railroad gate on the Northeast end
of the project and to any improvements that may be in progress at the west end
of the project. LPV 111.02, and the Inner Harbor Navigation Canal floodwall.
Evaluation
will be based on the Tradeoff Process (Best Value Continuum) in accordance with
FAR SubPart 15.101-1 and Source Selection SubPart 15.3. The evaluation criteria
consists of four (4) Technical (non-cost) factors, each of which has additional
sub-factors for evaluation and Price. The Technical (non-cost) factors, listed
in descending order of importance, are: (1) Technical Experience; (2) Past
Performance; (3) Preconstruction Services Plan; and (4) Construction Approach.
Technical (non-cost) factors when combined are significantly more important
than Price. The basis of award will be made to the Offeror who provides the
best value to the Government. The resulting award will be a Firm Fixed Price
base portion for the preconstruction services and Fixed Price Incentive Option

Extent of

The U.S.
Army Corps of Engineers (USACE), Hurricane Protection Office intends to issue a
solicitation for preconstruction services and construction option(s) for Levee
Improvements to the Chalmette Loop Levee, Reach LPV 146 in St. Bernard Parish,
LA. The solicitation will be a Request for Proposal for a Firm Fixed-Price Base
and Fixed Price Incentive requirement in accordance with FAR SubPart 16.403
utilizing Early Contractor Involvement (ECI) also known as Construction Manager
at Risk. The project is critical to the Hurricane Storm Damage Risk Reduction
System (HSDRRS) and must be completed by June 1, 2011.
The levee
system is an essential component of the New Orleans HSDRRS in St. Bernard
Parish. LPV 146 is a levee segment located between LPV 144 (Bayou Dupre) and
LPV 147 (Highway 46 and Bayou Road Floodgate), from station 708+65 to 1116+71,
approximately 40,800 LF. The existing levee elevation is approximately 20.0,
and was constructed as part of Task Force Guardian. The project will construct
a new T-wall to elevation 29.0. The new walls will tie in to the LPV 144 (Bayou
Dupre) sector gate structure on the west at approximate station 708+65, and to
the LPV 147 (Highway 46 and Bayou Road Floodgate) on the east at approximate
station 1115+30. This reach includes 2 pipelines that will require relocation.
Evaluation
will be based on the Tradeoff Process (Best Value Continuum) in accordance with
FAR SubPart 15.101-1 and Source Selection SubPart 15.3. The evaluation criteria
consists of four (4) Technical (non-cost) factors, each of which has additional
sub-factors for evaluation and Price. The Technical (non-cost) factors, listed
in descending order of importance, are: (1) Technical Experience; (2) Past
Performance; (3) Preconstruction Services Plan; and (4) Construction Approach.
Technical (non-cost) factors when combined are significantly more important than
Price. The basis of award will be made to the Offeror who provides the best
value to the Government. The resulting award will be a Firm Fixed-Price Base
portion for the preconstruction services and Fixed Price Incentive Option for
the construction phase.
Extent of
participation of small businesses (SB), small disadvantaged businesses (SDB),
women-owned small businesses (WOSB), veteran-owned small businesses,
service-disabled veteran-owned small businesses, HUBZone small businesses, and
minority institutions on the proposed contract are measured as a percentage of
the total planned subcontracting dollars. The selected firm, if a large
business firm, must comply with FAR Subpart 52.219-9 regarding the requirement
for submitting and negotiating a subcontracting plan. The subcontracting goals
for this District are: (1) at least 70.00% of the subcontract amount be placed
with SBs, which include SDBs and WOSBs; (2) at least 6.2% of the subcontract

149 (Verret to Caernarvon Floodwall and Structures) on the east at approximate
station 1552+50. This reach includes the St. Marys Pump Station No. 8, which
will require construction of new fronting protection to elevation 30.5; removal
of the existing Creedmore Drainage Structure; and relocation of 12 pipelines.
Evaluation
will be based on the Tradeoff Process (Best Value Continuum) in accordance with
FAR SubPart 15.101-1 and Source Selection SubPart 15.3. The evaluation criteria

of the loop levee is only accessible by boat. The existing levee elevation is
approximately 20.0, and was constructed as part of Task Force Guardian. The
project will construct a new T-wall to elevation varying from 31.5 to 29.0. The
new walls will tie in to the Inner Harbor Navigation Canal Lake Borgne Barrier
on the west at approximate station 405+00, and to the LPV 144 (Bayou Dupre)
sector gate structure on the east at approximate station 703+98.The reach
includes six gas pipelines that will require relocation.
Evaluation

The U.S.
Army Corps of Engineers (Corps) has authority to place a priority rating on its
contracts for industrial resources, in accordance with the Defense Priorities
and Allocations System (DPAS) regulation (15 C.F.R. Part 700), to support its
Hurricane and Storm Damage Risk Reduction System (HSDRRS) program in order to
ensure the timely completion of this hurricane protection system by June 2011
for the greater New Orleans area. On 29 October, 2008, Department of Homeland
Security/Federal Emergency Management Agency determined the HSDRRS program as a
critical infrastructure protection and restoration activity
The
HSDRRS program has been authorized the use of a DO priority rating. In that
capacity, this notice is to inform contractors that this is a DO-N7 rated
orders will be iussed under the authority of the Defense Priorities and
Allocations System (DPAS) regulation which is administered and enforced by the
U.S. Department of Commerce. A copy of the regulation can be found at
www.bis.doc.gov/dpas.
A free

System.
The priority
rating on the USACE order indicates that:
1. The
order supports an authorized national defense program. The DO indicates that
this program is vital to the national defense, and the N7 specifies the program
(critical infrastructure protection and restoration).
2. This
rated order contains a statement that it has been certified for national
defense use and you are required to follow all the provisions of the DPAS
regulation (15 C.F.R. 700).
3. The
order contains an authorized written or digital signature.
4. The
required delivery date is indicated in the order.
Receipt
of a rated order requires the contractor to do four things:
1.
Mandatory Acceptance/Rejection: A company in the Unites States must accept and
fill a rated order for items that the company normally supplies. A company may
not discriminate against rated orders in any manner, such as by charging higher
prices or by imposing different terms and conditions than for comparable unrated
orders. The existence of previously accepted unrated orders or lower rated
orders is not sufficient reason for rejecting a rated order. A company may
reject a rated order for delivery on a specific date if unable to meet that
date. However, the company must inform the customer of the earliest date on
which delivery can be made and offer to accept the order on the basis of that
date.
2.
Customer Notification Requirements: A company must accept or reject a rated
order and transmit the acceptance or rejection (in writing or electronically)
to the customer placing the order within fifteen (15) working days after
receipt of a DO rated order. If the order is rejected, the company must give
reasons in writing (not electronically) for the rejection.
3.
Preferential Scheduling: A company in receipt of a rated order must schedule
operations, including the acquisition of all needed production items, in at
timely manner to satisfy the delivery requirements of each rated order
4.
Extension of Priority Ratings: A company who receives rated orders must in turn
place rated orders with its suppliers for the items needed to fill the rated
orders. This continues from contractor to subcontractor to supplier throughout
the entire supply chain.
USACE
intends to ensure that the HSDRRS stays on schedule. Accordingly, the New
Orleans District Contracting Division will present a briefing for interested
industry members regarding the DPAS priority rating on 25 March 2009 at 2:00 pm
in the District Assembly Room, 7400 Leake Ave., New Orleans, 70118.-3651 Please

to attend the presentation which will be approximately one and half hours in
duration.

in this critical national defense program will help protect and enhance our
nations vital interests.
